NOTE: your responses are anonymous. This quiz is intended to demonstrate if you could be an initial candidate for the RELIEVE-HF trial. Eligibility is determined by a number of clinical and personal factors, which should be discussed by you and your physician.

#1. Are you age 18 or older?

#2. Have you been clinically diagnosed by a physician with Heart Failure?

#3. Are you currently taking medication for Heart Failure?

#4. Have you recently experienced symptoms for Heart Failure, including shortness of breath, fatigue, inability to lie flat or reduced capability for exercise or activity?

#5. In the past 12 months, have you been hospitalized or treated in an ER department for Heart Failure?

#6. Are you currently on kidney dialysis?

#7. Are you currently enrolled in another clinical research study for an investigational drug or medical device?

#8. Have you ever had a heart transplant or are currently awaiting one?

#9. Do you currently have an implanted pacemaker or defibrillator to treat your heart rhythm?

#10. Do you currently have an implanted mechanical heart assist (pump) device?
